Our Solutions
Practical Solutions for a More Connected Healthcare System
OnDemand Visit helps patients, families, and partners address common care challenges, including low-acuity urgent needs, avoidable emergency department use, rising-risk patients, missed follow-up, social barriers, rural access gaps, and confusion about where to go for care.
Our solutions are designed around the moments when people need extra support, whether that means responding to an urgent but non-emergent concern, identifying and enrolling high-risk patients before needs escalate, supporting a safe transition home, or connecting people to resources that help them remain stable at home.

Urgent Care Response
Not every urgent health concern requires an emergency department visit. For low-acuity and rising-risk patients, timely in-home or virtual support can help clarify symptoms, provide guidance, and determine the most appropriate next step.
OnDemand Visit connects patients with EMS-supported response and virtual nurse practitioner guidance when clinically appropriate, helping assess needs, review medications, provide education, recommend next steps, and determine whether escalation is needed while supporting care in the home whenever safe and appropriate.
Proactive High-Risk Outreach and Enrollment
Some patients need support before a crisis occurs. OnDemand Visit helps partners identify, reach, and enroll high-risk and hard-to-engage patients who may benefit from additional clinical guidance, social support, and care coordination, while helping patients feel seen, supported, and connected earlier.
Through proactive outreach, patient engagement specialists, EMS touchpoints, in-home visits, virtual clinical support, and connection to health plan, provider, and community resources, OnDemand Visit helps patients enter the right program earlier and stay connected to support that can reduce avoidable utilization and improve stability.

Transitions of Care
The days after discharge can be overwhelming. Patients may be managing new medications, follow-up appointments, changing symptoms, discharge instructions, or uncertainty about when to seek help.
OnDemand Visit supports patients after hospital or facility discharge through outreach, in-home visits, medication review support, education, virtual nurse practitioner access, and follow-up coordination. This helps patients better understand their care plan and gives partners greater visibility into needs that may surface after the patient returns home.
Quality Measures & SDOH Gap Closure
Health outcomes are shaped by more than medical care alone. OnDemand Visit helps identify and address barriers such as transportation needs, medication concerns, food insecurity, missed follow-up, home safety issues, behavioral health needs, and other social drivers that may affect a patient’s ability to stay well.
By bringing screening, education, and resource connections into the home, we help our partners close gaps in a way that is personal, practical, and actionable.

Rural Reach
Patients in rural and hard-to-reach communities often face long distances, transportation barriers, provider shortages, and limited access to timely follow-up care.
OnDemand Visit leverages locally embedded EMS teams and virtual nurse practitioner access to extend support into communities where patients may otherwise have limited options. This helps partners reach more patients while supporting care delivery across urban, suburban, and rural settings.
