Our Programs

Four Powerful Programs. One Seamless Solution.

OnDemand Visit’s programs turn common healthcare challenges into structured, scalable care models. Each program supports a specific need, including urgent care response, high-risk outreach and enrollment, transitions from facility-based care to home, 9-1-1 navigation, care gap closure, and ongoing support.

ODV Primed℠

Bringing Urgent Care Directly to the Patient Home

ODV Primed is an in-home urgent care response model designed for low-acuity and rising-risk patients who need timely support but may not require an emergency department visit.

Through EMT and paramedic field response supported by nurse practitioner telehealth oversight, ODV Primed provides symptom assessment, treatment recommendations, education, escalation guidance, and follow-up coordination. This model helps patients receive care at home when appropriate while helping hospitals and payors reduce avoidable ER utilization and admissions.

ODV CareLINC℠

Identifying, Engaging, and Enrolling High-Risk Patients

ODV CareLINC supports patients with complex medical, behavioral, and social needs who may be high-risk, hard to reach, or frequently reliant on EMS or the emergency department.

The program combines proactive identification, outreach, assessment, enrollment, and ongoing monitoring. Nurse practitioners, paramedics, social workers, community health workers, and patient engagement specialists work together to address barriers, close care gaps, connect patients to resources, and support long-term stability.

ODV Guide℠

Guiding Patients from Hospital to Home

ODV Guide supports patients during the critical period after discharge from a hospital, skilled nursing facility, or other care setting.

The program helps patients understand discharge instructions, review medications, identify barriers, connect with follow-up care, and access clinical guidance when questions arise. Through in-home visits, virtual nurse practitioner support, remote monitoring, and follow-up coordination, ODV Guide helps create smoother, safer transitions from facility-based care to home.

ODV Pathways℠

Treat-in-Place / 9-1-1 Navigation

ODV Pathways℠ gives EMS teams and community partners additional options for low-acuity 9-1-1 encounters when a patient may not require transport to the emergency department. When clinically appropriate, patients can be connected with OnDemand Visit support, including care navigation, virtual nurse practitioner access, urgent care response, proactive follow-up, and resources that help guide them to the right level of care.

This approach keeps patient safety at the center while helping partners reduce avoidable emergency department utilization, preserve emergency resources for higher-acuity needs, and provide a clearer care plan for patients who may be better served outside the emergency department.
