Our Partners
Helping Partners Engage Patients Differently
OnDemand Visit partners with hospitals, health systems, health plans, EMS systems, fire departments, providers, and community organizations to extend care beyond traditional settings and help patients navigate the system with greater confidence.
Our model is designed for partners who want to support patients in a more personal, proactive, and practical way while also improving outcomes, strengthening quality performance, reducing unnecessary utilization, and creating more efficient ways to provide care outside the hospital or clinic.

Partner Pathways

Hospitals and Health Systems
OnDemand Visit helps hospitals and health systems extend care beyond the hospital by supporting low-acuity urgent needs, improving follow-up after discharge, supporting smoother transitions, identifying barriers earlier, and connecting patients with the resources they need to recover, remain stable, and avoid unnecessary emergency department visits or readmissions.
Health Plans
OnDemand Visit helps health plans engage members who may be high-risk, rising-risk, hard to reach, recently discharged, or in need of support closing quality and social needs gaps. Through EMS-powered outreach, urgent care response, in-home and virtual care, proactive enrollment, and connection to plan and community resources, OnDemand Visit helps members access the right care pathway before needs escalate.

EMS Systems and Fire Departments
OnDemand Visit helps EMS systems and fire departments expand their role in the healthcare continuum by providing additional pathways for low-acuity calls, urgent care response, treat-in-place support, care navigation, proactive outreach, and follow-up resources. This helps preserve emergency resources while giving patients more appropriate options for non-emergent needs.
Community Organizations
OnDemand Visit works with community organizations to help identify barriers, connect patients with local resources, and support needs that extend beyond the clinical setting. These partnerships help strengthen the support network around patients and improve long-term stability.
